  • Please refer to the governing regulations on your application to ensure that you are eligible.
     
  • See the District section under "Auditions" to determine the appropriate mailing address for your district representative.
     
  • Applicants must include the following:

    1. Completed application form (typewritten or legibly printed) with candidate's signature, sent directly to the appointed auditions representative on or before the established deadline. (Unsigned or incomplete applications will not be accepted.)

    2. Certified copy of candidate's birth certificate or other legal proof of age such as driver's license or passport. Age requirements for 2007-2008: Birthday must fall on or between August 1, 1977 and July 31, 1987.

    3. Non-refundable application fee of $30 in the form of a check made out to:  "MONC/<your district>"

    4. An 8x10 black and white photo is requested, if available.
